Free Parking
We are located on the third floor right above Starbucks. There are plenty of parking spaces around the premise but more spots are available in the rear parking lot across Santander Bank. You will be entering the second floor of the building if using the rear entrance facing the bank. If you park in front of Starbucks, you can access the building through the side entrance between Starbucks and Bank of America ATM and then take the elevator up to the 3rd floor. Your first Initial Consultation and Acupuncture treatment will be approximately 1.5 hours in length. You will need to fill out the Health History Form and bring it with you completed to your session. If you are not able to do this at home, please come to your appointment 15-20 minutes early to fill them out at the office.
Please read the Arbitration Agreement & Acupuncture Informed Consent to Treat before you come.
It is necessary to sign the two forms (1 page for each) if you like to receive our treatment.Please wear or bring loose and comfortable clothing.
